LIMITED LIABILITY

This is an agreement between the client and Bohdan Guerra (the provider) concerning the Realization Session. 
 

1. I authorize and request Bohdan Guerra to carry out a Realization Session. I understand the process of this session will be explained to me upon my request and that they are subject to my agreement. I also understand that while the course of my session is designed to be helpful, Bohdan Guerra can make no guarantees about the outcome of my session. Further, this process can bring up uncomfortable feelings and reactions such as anxiety, sadness, and anger. I understand that this can be a normal response to this method of nondual inquiry and will address these reactions with Bohdan if they arise.

2. It is my responsibility to notify the provider of any current medical conditions and psychiatric diagnoses.

I understand that the services provided by Bohdan Guerra are intended to help the client within the context of spirituality, and any medical or psychiatric issue or condition that occurs during or after the Realization Session is not the responsibility of Bohdan Guerra. In addition, Bohdan Guerra will not be held liable for any information withheld by me as to my medical or emotional conditions. I will not hold Bohdan Guerra responsible or seek compensation for any injury or illness suffered by me caused in whole or in part by my participation in this session.
 

3. Receiving a Realization Session will not interfere or replace traditional medical or psychiatric care but can enhance other medical/psychiatric treatments. Therefore, clients under current medical or psychiatric care should not stop treatments or medication without advice of their physician/psychiatrist. By submitting this form, the client understands that Bohdan Guerra is not a medical doctor or licensed psychotherapist.
 

4. For maximum benefit, it is recommended that you do not consume any alcohol or drugs 12 hours prior and 12 hours after a Realization Session.
